Monday – Thursday: 8:00am – 5:00pm
Fridays: Procedures by appointment only
Appointments are an important part of your treatment, and this time has been reserved specifically for you. Please arrive 15 minutes prior to scheduled appointment time.
Late Policy: Patients arriving within their scheduled appointment time will be checked in as normal. Patients arriving 1–15 minutes late may be seen at the provider’s discretion or asked to reschedule.
No Show Policy: We understand that things do arise but after the third (3) missed appointments, a letter will be sent to our patients stating their relationship is in jeopardy. Upon additional "no shows" the patient will be dismissed from the practice and the appropriate letter will be mailed to them.

We accept Medicare and Medicare Advantage plans, along with many major commercial insurance plans, including BlueCross BlueShield (excluding Networks E and L), Cigna, UnitedHealthcare (UHC) commercial plans, and most Aetna plans. We also accept workers compensation cases with provider and adjuster approval. If your insurance is not listed, please contact our office. We are happy to help verify your coverage.
While a referral from your primary care provider is often preferred, patients may contact our office directly to inquire about scheduling an appointment.
Patients must be seen in the office for medication refills. If you have a question or concern about your medication, you may call the office to speak with a nurse. If a nurse is not immediately available, you may leave a message and your call will be returned.
